LPN Care Coordinator - REMOTE, Advanced Care at Home
Base pay range
$28.05/hr - $42.10/hr
Salary up to $42.10 per hour

What To Expect (Job Responsibilities)
Coordinate healthcare services for patients in the Advanced Care at Home program, including triaging clinical inquiries and assisting with patient monitoring
Facilitate effective communication between patients, internal teams, and external service providers
Manage the referral process for appropriate departments, ensuring accurate data entry and compliance with insurance requirements
Conduct patient outreach to provide support, education, and disease management instruction
Monitor and generate reports for effective caseload management
What Is Required (Qualifications)
Licensed Practical Nurse (LPN) license issued by the state of Illinois
Completion of an accredited or approved program in Practical Nursing
Typically requires 3 years of experience in an LPN role
Excellent telephone, interpersonal, and written communication skills
Strong knowledge of Medicare rules and regulations
